Where is ACTive Family Physical Therapy Located?

ACTive Family Physical Therapy has 2 locations.

Both Pediatrics and Performance are located within Crossfit 904 off of Phillips Highway.

Do I see the same therapist each time I come in?

Yes! At ACTive Family we believe it is important for the therapist and patient to have a great working relationship. If you are jumping back and forth between therapist it is hard to make session to session adjustments based on the previous sessions response to therapy

Do you take my insurance?

ACTIve Family Physical Therapy believes in upfront billing without surprises. This ensures you know how much you are paying each session and helps keep healthcare costs low for you.   Because of this, we operate as an out-of-pocket payment provider. This allows us to eliminate the possibility you will receive a larger bill after your insurance is billed and to reduce administrative costs that are transferred to the client when billing your insurance larger amounts.  This method typically results in fees that are lower than your out-of-pocket cost when billing directly through insurance depending on your deductible and co-responsibility.  This means what you pay at each treatment is the only amount you will be billed

ACTive Family can also provide you with a Superbill upon request that you may submit to your insurance company to request reimbursement. While many individuals successfully obtain part or full reimbursement, we cannot guarantee this to our patients. 

ACTive Family Physical Therapy also accepts payments through Step Up For Students Scholarship Funds for our Pediatric Patients.

How do I pay for my sessions?
All payments are due at the time of service. We accept cash, check, and credit card. Since we are a medical provider, you are also eligible to use your HSA or FSA for payment.
What is your cancellation policy?

Appointments canceled 24 hours in advance will be issued a full refund. Appointments canceled within 24 hours will be credited toward a future visit.  No show appointments will forfeit the amount of the session in full.
