Services

ACTive Family Physical Therapy offers physical therapy sessions, injury screenings, manual therapy, dry needling, IASTM, cupping, post-operative rehab, and rehab-to-performance training in Jacksonville and Ponte Vedra, Florida.

All appointments must be scheduled in advance. Payment is due at the time of service unless otherwise agreed upon. We are a cash-based provider and do not bill insurance.

Cancellation Policy

We ask for at least 24 hours' notice for cancellations. No-shows or late cancellations may result in a fee.
